    What is psychiatric medication management?

    Medication management is an outpatient treatment that involves initial Comprehensive Psychiatric Evaluation to determine Provisional Diagnosis and Treatment Plan. If appropriate, Psychotropic medications are prescribed. Laboratory studies are ordered if needed. Referrals to other specialists for underlying medical conditions may be made.

    Talk therapy may be recommended.

    Patients leave the initial Evaluation with a follow up Medication Management Visit within 2 to 4 weeks usually.

    During Follow Up visits, the psychiatrist will review compliance with prescribed medications and inquire about any side effects, concerns, or questions. Laboratory results may be reviewed. Information from other medical providers may be reviewed. Any concerns of the therapist will be addressed. Drugs or Alcohol use/ abuse will be assessed, and appropriate interventions are made. The psychiatrist will assess any benefit of medications by interviewing and observing the patient’s behavior, attitude, and mental state.

    Finally, a decision is made to continue, adjust doses, or change medications to achieve the treatment goals.

    Ongoing medication monitoring by a psychiatrist/prescriber is mandatory for safe and effective use of the psychotropic medications and to reach your treatment goals.

    WHAT IS MTM?

    Community Pharmacists provide Medication Therapy Management {MTM} for best therapeutic outcomes, to minimize side affection and drug- drug interactions.

    MTM includes five core elements:

    • medication therapy review
    • a personal medication record
    • a medication-related action plan
    • intervention or referral
    • documentation and follow-up
